Why are medicines so expensive in the US?

Insulin provides a lifeline for Type One diabetics. It costs less than $6 (£4.74) to make one vial. And yet in the USA, the average customer must pay $99 (£78.20) to access it. The result is that some point just cannot get hold of lifesaving medication.
